Course participants must be at least 12 years of age upon the start of the class.

As seen on your passport

NB: The total size of all uploaded files may not exceed 20 MB.

We accept National ID, UMID, NBI clearance, Passport, PRC, and Drivers License.


IMPORTANT

  • Filling out the form does not guarantee a course slot. 
  • This online mask must only be filled by the course participant.
  • Remember to double check your registration form before submitting to make sure that the desired dates and time are correct. 
  • You are only allowed to fill-up and send this registration form once. We don't accept multiple registrations.
  • After filling out the form, please wait for the acknowledgement receipt in 3-5 business days.
